Important Information about Mail Order Prescription (RX) Services
Nishat Hospital offers convenient mail order prescription (RX) services for eligible patients, ensuring timely delivery of prescribed medications directly to your home. This service helps maintain treatment continuity, especially for long-term therapies. Patients must provide a valid prescription, current contact details, and delivery address.
Please plan to arrive at least 15 minutes ahead of time to fill out any paperwork. Please bring the following:
- Insurance card
- Government-issued identification card
- Prescription card
- Co-payment if your insurance plan requires it
- A list of your current medications
- On your first visit, you may be asked to sign a (HIPAA) privacy form.
Regarding Your Insurance Deductibles
At Nishat Hospital, patients using insurance are responsible for any deductibles as per their policy terms. Your deductible is the amount you must pay out of pocket before your insurance coverage begins. Our billing team will inform you of the exact deductible amount in advance and guide you through the payment process, ensuring clarity and transparency in your medical expenses.
